The Bile-Hormone Highway: Why Your Liver Is Driving Your Acne

The first article in this series ended with a claim: that acne is what happens when your liver doesn't have the raw materials to clear hormones, those hormones back up into circulation, and your skin reads the surplus as a signal to produce more sebum.

This article is the inside of that mechanism. It is, in my view, the chapter the dermatology curriculum should start with and instead never covers. If you're going to understand why your skin behaves the way it does — and why most acne interventions miss — you have to understand the highway your body uses to take used hormones out of circulation.

This is that highway. It has a specific anatomy, a specific failure mode, and a specific intervention. None of it requires you to be a medical professional to understand. All of it is in the peer-reviewed literature, just scattered across journals dermatologists don't read.

The route, in one paragraph

Hormones — testosterone, estrogen, cortisol, and their metabolites — are produced or absorbed, circulate through the bloodstream, do their work, and need to be removed. The liver is where removal happens. The liver attaches a molecular "tag" to each used hormone in a process called conjugation, which makes the hormone water-soluble. The conjugated hormone is then excreted into bile. The bile flows from the liver into the small intestine. The hormone, bound to the bile, moves through the intestine and out of the body via stool.

That is the entire route. When it works, it is invisible. Hormones cycle. The skin behaves. Most of the people you know who have always had clear skin without thinking about it have this pathway working at full capacity, even though they have never heard of it.

When it fails — at any point along the route — the hormones back up. And the place hormone surplus shows up most visibly is the skin.

Step one: Phase II conjugation

The first step happens inside the liver. Phase II conjugation is the umbrella term for six different chemical processes the liver uses to neutralize and tag toxins, drugs, and used hormones for elimination. The six pathways are: glucuronidation, sulfation, glutathione conjugation, methylation, acetylation, and amino acid conjugation.

For acne, two of these matter most: sulfation and glucuronidation. Both are responsible for tagging used hormones — particularly androgens and estrogen metabolites — for excretion.

Here's the part nobody told me until I was forty: sulfation depends on sulfur. Specifically, on a sulfur-containing molecule called PAPS (3'-phosphoadenosine-5'-phosphosulfate), which is the universal sulfate donor inside cells. Your body manufactures PAPS from the sulfur-containing amino acids in your diet — methionine and cysteine — and from inorganic sulfate.

If your sulfur substrate is low, your PAPS pool is low. If your PAPS pool is low, sulfation slows down. If sulfation slows down, hormones that need to be cleared by sulfation start backing up. And the modern diet, as I covered in the first article, has stripped sulfur out aggressively over the past century.

This is why people who eat lots of raw cruciferous vegetables, egg yolks, and organ meats — all sulfur-dense foods — tend to clear acne when they shift toward that pattern. They are not "eating clean." They are restoring sulfur substrate for Phase II conjugation. The skin clearing is downstream.

Step two: bile flow

Once the liver has conjugated the hormone, it needs to send it out. The vehicle is bile.

Bile is produced in the liver, stored in the gallbladder (or, if you've had your gallbladder removed, secreted continuously into the small intestine), and released into the duodenum — the first segment of the small intestine — in response to fat ingestion. Bile has multiple jobs: it emulsifies dietary fats for absorption, it neutralizes stomach acid as it enters the small intestine, and — relevant here — it carries conjugated waste products, including used hormones, out of the body.

For bile to flow properly, several conditions have to hold:

  • The liver has to be producing enough bile
  • The bile ducts have to be open and unobstructed
  • The gallbladder has to be contracting normally (or, post-cholecystectomy, continuous flow has to be adequate)
  • The duodenum has to be sufficiently alkaline for bile to function (bile is alkaline; it needs an alkaline environment to do its work)
  • The pancreas has to be releasing bicarbonate into the duodenum to maintain that alkaline pH

Any of those can fail. When they do, bile flow stalls. Bile sludges. Bile becomes thick and sluggish. The conjugated hormones the liver worked to package for elimination don't move. They accumulate.

The most common failure point in the modern context is the last one on the list: pancreatic bicarbonate output. Bicarbonate is what makes the duodenum alkaline. Without enough bicarbonate, the duodenum is too acidic, bile doesn't function properly, and the entire downstream chain breaks.

Step three: intestinal transit

Assuming bile flow is normal, the conjugated hormone is now in the small intestine, moving through the digestive tract toward elimination. This step seems straightforward — and for most healthy people, it is — but it has two specific failure modes worth knowing about.

The first is transit time. If you are chronically constipated, conjugated hormones spend more time in contact with the intestinal lining than they should. Some of them get reabsorbed back into circulation through a process called enterohepatic recirculation. The hormone you just worked to eliminate gets pulled back in.

The second is the gut microbiome. There is a specific class of bacteria in the gut that produces an enzyme called beta-glucuronidase. This enzyme un-tags conjugated hormones — it breaks the bond the liver attached in Phase II — and renders the hormone reabsorbable again. When the gut microbiome is dysregulated and beta-glucuronidase activity is elevated, you can have a working liver and working bile flow and still have hormones reaching your skin because they're being un-conjugated in the gut and recycled back.

This is why "gut health for skin" is real but most people get the mechanism wrong. The gut isn't producing the acne. The gut is failing to eliminate what the liver already worked to remove.

Where the highway most commonly fails

If you map the failure points by frequency in modern adults, the picture looks like this:

  • Phase II sulfation slowdown from sulfur substrate deficiency — affects most adults to some degree
  • Pancreatic bicarbonate insufficiency — common in people with mineral deficiencies, chronic stress, or proton pump inhibitor use
  • Bile sludge or thick bile — common in people on low-fat diets (the gallbladder rarely contracts and bile becomes static), in pregnancy, and in those with hormonal imbalances
  • Constipation — extends gut transit time and increases enterohepatic recirculation
  • Elevated beta-glucuronidase activity in the gut microbiome — common in diets low in fiber and high in processed food

The pattern most people present with is some combination of all five. Not one acute failure, but a chronic low-grade slowdown across the whole route, each piece contributing a fraction. Address one piece and you get partial results. Address the route — feed the substrate, support the alkaline environment, keep transit moving — and the whole system resumes function.

The intervention this points toward

Once you understand the highway, the intervention almost designs itself. You need to:

  1. Restore Phase II substrate — give the liver the sulfur it needs to perform conjugation
  2. Support the alkaline duodenum — supplement the bicarbonate the pancreas may not be producing enough of
  3. Provide the cofactor minerals — the enzymes that do Phase II conjugation are mineral-dependent; without the cofactors, the substrate sits unused

That is the architecture of the protocol I built for my son. Three inputs. Each one targets a specific step in the highway. Together, they restore flow.

What This Reframes

If acne is downstream of stalled hormone clearance, and hormone clearance depends on a specific anatomical chain — Phase II conjugation in the liver, bile flow into the duodenum, normal intestinal transit — then "treating acne" topically is treating a symptom three steps removed from the problem. The interventions that work permanently are the ones that restore the chain.

What this is not

This framework explains a large share of persistent adult acne, hormonal acne, and stubborn cases that haven't responded to conventional treatment. It does not explain all acne in all people. A few important caveats:

  • Some acne is genuinely topical — acne mechanica from friction, cosmetic acne from comedogenic products, fungal acne (malassezia) that is actually a yeast not bacteria. These have different mechanisms.
  • Severe acute hormonal events — pregnancy, severe PCOS, hormonal medications — can overwhelm even a healthy clearance pathway temporarily.
  • True bacterial overgrowth in already-compromised skin can drive inflammation independently of the underlying hormone load.
